Service Address Location Information (SALI) Form

Overview

The Service Address Location Information (SALI) form is used to provide service address information and must be associated with one of the following forms:

• Feature Group A (FGA) Request
• WATS Access Line (WAL) Request
• Transport Request
• End User Special Access (EUSA) Request
• Ring Request

Description

The SALI form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Address Detail - identifies service address details
